Five Ways to Evaluate Mobile Medical Apps via Research | The ...

There are many barriers to the adoption of health, fitness, and medical apps. A recent article summarizing the report from PWC?s Global Healthcare division states that adoption of mHealth technologies will lag consumer demand. I wholeheartedly agree with David Levy, MD (global healthcare leader of PwC when he said ?Despite demand and the obvious potential benefits of mHealth, rapid adoption is not yet occurring. The main barriers are not the technology but rather systemic to healthcare and inherent resistance to change.?? Relevant to both of these is the lack of demonstration of clinical efficacy of medical apps. Few medical apps have obtained FDA approval. The demonstration of clinical efficacy and/or cost-savings is a key ticket to reimbursement which itself is significant with regards to adoption.? Proof that positive clinical efficacy trials lead to reimbursement is evidenced by Sotera Wireless? ViSi Mobile vital sign monitor and WellDoc?s DiabetesManager. ?Trials take time, are costly, not necessary unless falling under FDA requirements (yet to be announced), and not seen as important for business development by many app developers. Here are a few ways in which the trials might be done to demonstrate to payers, providers, and other stakeholders that medical apps should be adopted.

1.??? Crowdsourced research studies. While traditional clinical studies are the usual pathway to the healthcare marketplace for products like medical devices, crowdsourced health studies? are attractive for patient engagement necessary tools like medical apps.? Online health communities like wegoHealth, patientslikeme, 23andme, and others are groups of patients and caregivers with common diseases and concerns who might participate in crowdsourced studies. The merits and drawbacks of crowdsourced health studies have been discussed in more detail in a previous post.

2.??? Academic institutional-based studies.? Medical schools are developing their own apps.? There are multiple reasons for this phenomenon.? Johns Hopkins University School of Medicine has been developing apps as well as ?performing clinical efficacy studies on selected health apps. There has been long-time collaboration between industry and academia.? I do not see why medical apps and other digital health technologies cannot follow this path.

3.??? Feasibility studies. ?As adoption has been slow and not many apps are undergoing clinical trials, perhaps a good first ?baby step? is to conduct feasibility studies as Happtique is performing now on its medical app prescribing program called mRx. ?The study will not evaluate efficacy of apps prescribed by physicians but will evaluate how many times physicians prescribe the app and how many times patients fill them. While feasibility studies are not efficacy studies, they do spread awareness of the technology of prescribing apps, possibly improve patient-physician relationships, familiarize the parties involved with the process, and expose patients to what apps might do for them.

4.??? Comparative effectiveness studies.? Because of how many apps there are within specific areas of health and medicine, they might lend themselves to open label trials without necessarily a standard care study arm.

5.??? Other unique research study design methods.? In August 2011, scientists, policy makers, technologists, health professionals and others gathered at the mHealth Evidence Workshop to discuss new and unique methods of evaluating efficacy of mHealth technology. The diversity of technologies, study populations, and the rapidly changing landscape of technology lends itself to emerging ways of evaluating them.

Most stakeholders in the healthcare arena agree that apps will play, at some point in time, a significant role in disease prevention and treatment.? Most apps will not fall under the requirement for FDA approval. Certification standards for usability, privacy, safety, and content have been developed by Happtique for their certification program which will begin soon.? Though this is itself a huge step in the evaluation process, these standards do not assess efficacy, leaving a large void.? Efficacy trials will be welcome by all, and need not be the same old clinical trials producing ?evidence-based? recommendations which have been the subject of recent attention.

Attacks on Palestinians kindle debate on values

JERUSALEM (AP) ? Two vicious attacks on Palestinians, presumed to be the work of Jews, have some Israelis worried that their society is increasingly tolerant of hate crimes.

Over the weekend, unknown assailants firebombed a Palestinian taxi in the West Bank. Hours later, a mob of Jewish teens beat a 17-year-old Palestinian unconscious in downtown Jerusalem.

Israeli leaders have condemned the violence. On Tuesday, President Shimon Peres said he was "mortified" by the "intolerable" attack on the teen.

Others are cautioning against making broad indictments of Israeli society, arguing that incidents of violence and intolerance are not widespread, despite constant tensions from the longstanding conflict between Arabs and Jews.

One suspect in the attack on the Arab teen hinted at the corrosive effect of that conflict when he declared outside court on Monday, "He could die for all I care ? he's an Arab."

The suspect was among seven people, aged 13 to 19, who have been arrested in connection with the assault on 17-year-old Jamal Julani. Bystanders watched as he was kicked and beaten, to the point that he was in critical condition when was brought into a Jerusalem hospital early Friday. He has since regained consciousness but remains hospitalized, a spokeswoman for Hadassah Medical Center said Tuesday.

Police spokesman Micky Rosenfeld said the violence was not the act of Jewish extremists, but "just kids" egged on by a Jewish girl who claimed to have been involved in an unspecified "sexual incident" with an Arab, an account her lawyer denied.

Rosenfeld said no arrests have been made in Thursday's firebombing of the Palestinian taxi, which wounded the six people inside. One remained in intensive care on Tuesday, and four others also remained hospitalized, Hadassah spokeswoman Etti Dvir said.

Prime Minister Benjamin Netanyahu vowed to bring the people responsible for the firebombing to justice and relayed that message to Palestinian President Mahmoud Abbas. His spokesman, Mark Regev, said the government "will have no tolerance whatsoever for racist violence."

Vice Premier Moshe Yaalon described the two weekend attacks as "terrorist acts" that "run contrary to Jewish ethics and values."

Critics charge that authorities are lax in pursuing Jews suspected of attacking Palestinians, noting few indictments or convictions in dozens of cases of assaults on Palestinians and their property every year.

They also accuse some of Israel's political and spiritual leaders of creating a toxic atmosphere by condoning ? and even encouraging ? violence against Arabs.

"Even the denunciation of the prime minister and other public figures can't obscure the fact that this (attack on the Arab teen) has a deep-seated political and social context," the Haaretz newspaper editorialized on Tuesday.

Social critics note that the decades-long conflict between Israel and the Palestinians has bred hatred, dehumanization and violence on both sides. Palestinian attacks on Israeli vehicles in the West Bank are frequent, and many Palestinians consider them legitimate forms of resistance against Israeli occupation.

The Israeli military counted 196 Palestinian firebomb attacks and six shooting incidents in the first half of 2012, but it did not provide information on casualties.

Gerald Steinberg, a political science professor at Bar-Ilan University, said the incidence of violence among Israelis is not exceptional in the world context.

"There are pockets within Israeli society, particularly among young teenagers, where there is a history of violence, intolerance, across the board. It extends to Jewish groups in Israel as much as it does to Palestinians," he said. "The evidence clearly shows that Israel is not any more tolerant than the United States or Europe, and there is a lot of stress in this society."

Madagascar president says he tried to cut deal with rival

Madagascar's transitional leader Andry Rajoelina said Sunday that he had made a fruitless attempt to cut a deal with his rival, ex-president Marc Ravalomanana, in which neither man would run for office.

Speaking after his return from Mozambique, where he had attended a Southern African Development Community (SADC) summit aimed at solving the years-long standoff between the rivals, Rajoelina said he made the offer during a separate meeting with Ravalomanana, currently in exile in South Africa, in the Seychelles.

"I offered to not run if the former president agreed to return after the elections, but he did not want to," Rajoelina said.

SADC leaders said on Saturday they are considering presidential elections in Madagascar in May 2013 that would include neither Rajoelina nor Ravalomanana.

The 15-nation bloc suspended Madagascar from its ranks in 2009 after strongman Rajoelina toppled Ravalomanana.

Ravalomanana wants to return home to run for president but cannot do so for fear of prosecution after he was sentenced in absentia to life imprisonment over the killing of 36 protesters by presidential guards during unrest in 2009.

Various political groups penned a peace roadmap in September last year that includes provisions for Ravalomanana's return, but does not exempt him from prosecution.

The question of Ravolomanana's return from exile has been the main stumbling block to the holding of elections to end the three-year crisis on Africa's largest island.

The Seychelles has in recent weeks hosted two failed mediation attempts to get the two leaders to patch up their differences.

Sometimes, supervisors make dumb mistakes?for example, telling an employee that she won?t be transferred to another office because the people there don?t like co-workers of her ethnicity.

If you learn of such bone-?headedness, fix the problem fast. That could stop a costly lawsuit.

Recent case: Tomoko is of Japa??nese ancestry. She worked for a Japa??nese company in its Pennsylvania office. When management decided to close the office, it also decided not to transfer Tomoko to Detroit. She asked why. Her supervisor said the office didn?t want someone of Japa??nese ancestry working there.

Tomoko began looking for another job. Meanwhile, the company apparently decided it wasn?t smart to so nakedly admit to bias. It offered Tomoko the Detroit job, but she refused, quit and sued for discrimination.

The trial court dismissed her case, reasoning that she hadn?t been harmed and therefore couldn?t sue for national-origin or race discrimination. She appealed, but the 3rd Circuit Court of Appeals refused to reinstate the lawsuit. It, too, reasoned she had suffered no harm. (Fubayama v. Nichia American, et al., No. 11-2330, 3rd Cir., 2012)

Final note: If someone in your organization messes up like this, immediately set about repairing the damage. Then train supervisors on their equal employment opportunity obligations. You may also want to review recent employment decisions to make sure prejudice hasn?t played a part in discipline, hiring or discharge.

With Assange inside, British police guard Ecuador embassy

Britain warned it was "determined" to extradite Julian Assange as the WikiLeaks founder remained holed up Saturday in Ecuador's London embassy with police guarding all the exits.

Ecuador on Thursday granted asylum to Assange -- whose website enraged the United States by publishing a vast cache of confidential government files -- but Britain has vowed not to grant him safe passage out of the country.

British Foreign Secretary William Hague said his government was obliged under its own laws to extradite the Australian national to Sweden, where he is wanted for questioning over alleged sex crimes.

"No one, least of all the government of Ecuador, should be in any doubt that we are determined to carry out our legal obligation to see Mr Assange extradited to Sweden," Hague told a press conference.

The Organization of American States on Friday called a meeting of foreign ministers for August 24 to discuss the standoff.

Twenty-three countries voted for the resolution proposed by Ecuador to convene the meeting at its Washington headquarters. The United States, Canada and Trinidad and Tobago voted no.

The vote was taken at emergency talks to discuss the Assange case after Ecuador turned to the OAS for help.

The US envoy to the OAS, Carmen Lomellin, said a meeting of OAS foreign ministers "would be unhelpful and harmful to the OAS' reputation as an institution".

Ecuador wants the body to discuss Britain's threat to storm its embassy by force to take Assange into custody.

Under a 1954 agreement, the OAS agreed to allow asylum in diplomatic missions for "persons being sought for political reasons", although not individuals indicted for "common offenses".

The United States said earlier Friday it does not believe in the concept of "diplomatic asylum".

"We believe this is a bilateral issue between Ecuador and the United Kingdom and that the OAS has no role to play in this matter," the State Department said in a statement.

Australia confirmed Saturday that its diplomatic post in Washington had been preparing for Assange's possible extradition to the US but played it down as "contingency planning".

Trade Minister Craig Emerson said the Australian embassy in Washington had been "getting prepared for the possibility of an extradition" but stressed that there was nothing unusual in diplomats bracing for all eventualities.

"The embassy is doing its job, just to be in a position to advise the government if it believed that an extradition effort was imminent. There is no evidence of such an extradition effort," Emerson told ABC television.

"All that was happening is that the post in Washington was doing some contingency planning in the event that such an eventuality arose."

Meanwhile, both Ecuador and Sweden suggested Friday they were prepared to be patient and wait.

Ecuador's President Rafael Correa said Assange will remain in Quito's embassy as long as Britain refuses to give him safe passage.

"The problem is that they aren't going to give him the safe conduct," Correa said in a radio interview, adding that "Mr Assange can stay indefinitely in our embassy".

Swedish Foreign Minister Carl Bildt told Sweden's TT news agency: "We have no reason to do too much, we're going to wait and see what happens."

Some 20 British police were stationed outside the embassy Friday, ready to arrest the 41-year-old Assange if he leaves the premises.

WikiLeaks condemned the police presence as "intimidation tactics".

A handful of supporters of the former computer hacker camped overnight outside the embassy in London's plush Knightsbridge district in a bid to "guard" Assange against any potential attempt to arrest him.

"We'll stay here as long as we have to," 26-year-old protester Baba Gena told AFP.

A couple of activists brought a megaphone along and yelled at police: "Why aren't you doing your job properly?" and "Put your hands in the air if you believe in freedom of speech!"

Assange has not left the embassy since June 19, when he walked in and asked for asylum.

WikiLeaks said on Twitter that he would give a statement in front of the embassy on Sunday, though it did not specify whether this would involve leaving the premises and, if so, how he would do so without being arrested.

Under normal diplomatic procedures, embassies are considered the territory of the countries they represent and cannot be entered without permission.

Britain has angered Ecuador by suggesting it could invoke the Diplomatic and Consular Premises Act of 1987, which it says allows it to revoke the diplomatic immunity of an embassy on British soil and go in to arrest Assange.

But Philip Barton, Britain's envoy to the OAS as an observer, said: "Allegations that the UK was threatening Ecuador and about to storm the embassy are without foundation".

Baltasar Garzon, the renowned Spanish lawyer who is helping Assange's defence, has said the WikiLeaks founder will appeal to the International Court of Justice if Britain does not backtrack and guarantee him safe passage.

WikiLeaks spokesman Kristinn Hrafnsson told AFP that any bid to enter the embassy would "risk upsetting diplomatic relations all over the world".

A Foreign Office spokesman said Friday that Britain was "committed to working with the Ecuadorans to solve this matter amicably".

In 2010, WikiLeaks published hundreds of thousands of US military documents on the war in Iraq and Afghanistan, as well as diplomatic cables that deeply embarrassed Washington.

Supporters fear Assange could face the death penalty if he were to be sent to the United States and tried on espionage charges.

As we reported a few times this week, insurance company Progressive came under fire after Matt Fisher, the brother of a deceased policyholder, posted a scathing rant about the company?s refusal to honor her policy during the family?s post-accident claims. After the rant went viral, thanks to other bloggers, Progressive made a few public relations blunders in an attempt to mitigate the situation, leading to even worse blowback ? ?a PR catastrophe,? some might say.

Three days later, the company said it has reached a settlement with the Fisher family.

The family?s attorney, Allen Cohen, told CNN Money that it was a monetary settlement for ?tens of thousands of dollars? more than the $75,000 claim balance that the family was originally seeking.

The payment ?is for their failure to exercise good faith towards their insured,? Cohen said. ?While I cannot tell you the amount, they did pay 100 percent of what we asked in this regard.?

One unsatisfied commenter wrote on the Facebook page after the settlement was announced: ?Hey look at that, you agreed to pay off the claim the family was originally entitled to. All it took was losing a trial, a media firestorm, and the impending loss of countless customers.?

Search giant Google has taken another step in its quest to index all of human knowledge. This week, Google introduced a new section of its Google Patent search which allows anyone to search not only existing patents, but also prior art. Google?s introduction to its new Patent search:

?Typically, patents are granted only if an invention is new and not obvious. To explain why an invention is new, inventors will usually cite prior art such as earlier patent applications or journal articles. Determining the novelty of a patent can be difficult, requiring a laborious search through many sources, and so we?ve built a Prior Art Finder to make this process easier. With a single click, it searches multiple sources for related content that existed at the time the patent was filed.?

Jon Orwant, the head of Google?s Patent Search team, stated that web traffic to Patent Search has doubled recently, a trend that he attributes to an increase in high-profile, high-stakes patent litigation. ?People are thinking about patents a whole lot more,? he notes. One of the biggest examples of this type of patent litigation is the Apple v. Samsung battle. Hopefully, Apple and Samsung can use this search in the future to prevent them from stepping on each other?s toes, which could end up in billion-dollar lawsuits.

The new Patent Search does beg a couple of questions: As most lawyers are aware, one should not give legal advice to someone who is not a client, could this new search rise to the level of prior art? How much reliance may users place on Google?s efforts to index all patents and prior art? And most importantly, perhaps Google should do a search to makes sure there is no prior art for a prior art search. The irony would be almost too much to handle.

Hundreds interview for jobs at new Va. Beach Target ...

VIRGINIA BEACH

Ashwan Richards arrived at 8:30 a.m. Friday.

At 11 a.m., he was still waiting to interview for a job at the new Target store - one hopeful in a long line of applicants wrapped around Town Center.

"Gotta have a grown-up job. It's a grown-up world," said Richards, a 17-year-old headed into his senior year at Tallwood High School. This would be his second job; he said he needs the extra money to help his mother after his father lost his job.

He's not the only one who said times are tough.

More than 600 people interviewed Thursday at the Westin Virginia Beach Town Center for jobs at the discount department store, which opens Oct. 14 at Pembroke Mall.

By 10 a.m. Friday, another 160 had interviewed, said Shannon Roberts, a human resources leader opening the new store. Hundreds more waited in line.

Today is the third and final day of interviews. The store is hiring for 300 positions.

"It's the biggest turnout we've had in this area," Roberts said. "We're really excited."

Applicants showed up in khakis and T-shirts, skirts and suits. Some stood in the shadow of the Westin hotel; others waited across the street in the hot sun.

Around noon, Clay and Kathi Culbreth and their son Cody arrived with a trash can full of ice and bottled water. As they handed out the bottles to waiting job seekers, Kathi explained that her husband's office overlooks the long line.

"We were driving by and said, 'Let's get them some water,' " she said.

As they waited, people traded stories of lost jobs and bad jobs. Several, including Richards, said they have filed applications at businesses across the area.

Kimberly Landry of Chesapeake said she works at a grocery store, and recently her hours were cut. Even with her husband's income, it's not enough to pay their mortgage and support their 2-year-old, she said.

She hoped a new, better-paying job at Target could provide a boost.

"We live paycheck to paycheck," she said. "We are definitely struggling."

Randy Little of Virginia Beach said he lost a job in shipping and receiving when the company he worked for lost its biggest contract with a drugstore. The loss cost a lot of people their jobs, he said.

He, too, hopes to get a post at Target, maybe in its receiving or stocking departments, he said. But surveying the long line on Friday, he looked doubtful.

"It looks mighty bleak right now," he said.

All of that squabbling over H.264 may be rendered moot in the near future. The Motion Picture Experts Group (better known as MPEG) has just let us know that it was quietly drafting a new video standard while everyone was on summer vacation last month: H.265, also called High Efficiency Video Coding, promises to squeeze video sizes with double the efficiency of H.264. As you might imagine, this could lead either to a much smaller video footprint for bandwidth-starved mobile users or a hike to image quality with the same size as before. Imagine fast-loading HD streaming on 4G, or cable TV without all the excess compression, and you've got the idea. Ericsson Research visual technology lead Per Fröjdh anticipates H.265 coming as soon as 2013, when our smartphones and tablets are most likely to play it first. TV and other areas might have to wait, although Fröjdh is offering a consolation prize -- he's teasing a separate MPEG project that could give us glasses-free, compressed 3D video as a standard by 2014.

Astronomers find record-breaking galaxy cluster

(Reuters) - A massive so-called galaxy cluster, one of the largest structures in the universe, has been discovered about 5.7 billion light years from Earth and credited with setting several important new cosmic records, U.S.-based researchers said on Wednesday.

The Harvard-Smithsonian Center for Astrophysics said in a news release that observations of the cluster, which has shown a prodigious rate of star formation, may force astronomers to rethink how such colossal structures and galaxies that inhabit them evolve over time.

Known officially by an alphabet soup of numbers and letters as SPT-CLJ2344-4243, the cluster has been nicknamed "Phoenix," after the mythological bird that rose from the dead.

That's partly due to the constellation in which it lies. But Michael McDonald, a Hubble fellow at the Massachusetts Institute of Technology, said the Phoenix was also a great way of thinking about the latest astronomical marvel.

"While galaxies at the center of most clusters may have been dormant for billions of years, the central galaxy in this cluster seems to have come back to life with a new burst of star formation," said McDonald, the lead author of the paper on Phoenix, appearing in the August 16 issue of Nature.

Based on observations from NASA's Chandra X-ray Observatory, the U.S. National Science Foundation's South Pole Telescope and eight other observatories, researchers said the center of the Phoenix cluster had been linked to the creation of about "740 solar masses" or stars a year.

By comparison, the Perseus cluster forms stars at a rate about 20 times slower than Phoenix.

"This is just an enormous rate," said Marie Machacek, an astrophysicist at the Smithsonian Astrophysical Observatory. She said huge clusters like Phoenix are thought to host thousands of galaxies and there was still a lot to learn about what goes on within them.

Supermassive black holes in the central galaxy of a cluster have long been associated with low observed star formation rates, as they pump energy into the system and prevent the cooling of gases needed for the creation of stars.

But researchers said the "massive starburst" seen in Phoenix, as it gave birth to about two stars per day, suggested that its central galaxy's black hole had failed to interfere with an extremely strong cooling flow.

"Stars are forming in the Phoenix cluster at the highest rate ever observed for the middle of a galaxy cluster," the Harvard-Smithsonian Center for Astrophysics said in its press release.

"The object also is the most powerful producer of X-rays of any known cluster and among the most massive. The data also suggest the rate of hot gas cooling in the central regions of the cluster are the largest ever observed," it said.

Researchers develop DNA sequencing tests for hereditary diseases

ScienceDaily (Aug. 16, 2012) ? Scientists at the University of Medicine and Dentistry of New Jersey-New Jersey Medical School (UMDNJ-NJMS) have developed new DNA sequencing tests that hold significant promise for decreasing costs associated with diagnosing cancer and hereditary diseases, including cystic fibrosis.

Officials at the New Jersey Department of Health approved the use of the new Cystic Fibrosis (CF) Carrier and Diagnosis Test, which was created at the Institute for Genomic Medicine at UMDNJ-NJMS. Using a semiconductor mechanism that was developed by San Francisco-based Ion Torrent, the microchip tests the entire gene for mutations. IGM now offers this certified Clinical Diagnostic Laboratory service for hospitals as well as obstetrics and gynecology practices throughout the Garden State.

According to the Cystic Fibrosis Foundation web site, "More than 10 million Americans are symptomless carriers of the defective CF gene." This chronic disease impacts the lungs and the digestive system. It occurs when a child inherits one defective CF gene from each parent. Statistics show New Jersey averages 125,000 births of children who are diagnosed with cystic fibrosis annually.

"We believe the adaptation of this new sequencing technology will drastically improve our ability to analyze genetic disorders," said Marvin N. Schwalb, PhD, director of the Institute for Genomic Medicine. "Traditional CF sequencing testing costs thousands of dollars making the test unavailable for carrier screening. This new test costs less than $200. Most importantly, the genetic carrier test we developed improves the diagnosis rate to 98 percent. While the test provides significant improvement for all populations, the improved rate is particularly valuable for minorities because current carrier screening methods only detects approximately 65% of mutations in these populations."

The new technology provides many advances including the ability to test as many as 96 samples on a single platform and the fact that the equipment cost 1/10 as much as the previous technology.

IGM has developed another test, which was also approved by the NJHSS, for mitochondrial DNA. Mutations in mitochondria cause a wide variety of diseases, such eye and neuromuscular system disorders and possible cancer.

Schwalb, a professor of Pediatrics, Microbiology and Molecular Genetics at NJMS, said, "We are proud of the fact that the IGM is a world leader in the advancement of genetic diagnosis. DNA sequencing will keep us very busy for a while. In the state of New Jersey, there is nothing that compares to this advancement and this is just the beginning."
